Unchecked, fatalities can result just as this worker was killed two quarters ago, this accident was 100% preventable with proper engineering and administrative controls according to the investigation report issued this <a target="_blank" rel="noopener noreferrer nofollow" href="http://week.How">week.How</a> many trucks leave fabs every day hauling similar type of “water” and enter highways as pressurized vessels on the way to cement kilns, incinerators, and deep ground injection wells? Imagine sitting in traffic with this pressurized tank of acid waste and peroxide sitting next to you. Could it explode on a 100F degree day like it did when it killed this worker?How many tanks and pipes of “water” sit onsite at any given fab over pressurized? How many water samples get shipped in pressurized containers through USPS, UPS, FEDEX, DHL?I am astounded that a paltry $16K fine was levied to TSMC for this death, really shocked the root cause was not fully detailed in the report. What is most alarming to me is that this was originally reported as a refuse hauling truck (garbage) accident almost as if to cover up the fact it was a liquid waste hauling death. The communication on this death has been muted at best by the world leader of semi-conductor manufacturing.ADOSH found no violations of state or federal safety standards by United Pumping Service, the California-based company that lost their employee to the explosion.Will the OSHA report be more thorough and will the fine be commensurate with a negligent death? Captain 🧑‍✈️ obvious thinks the industry needs to socialize and learn from this death so it can prevent the next 🧐</p>]]></description>
